Continuity Care Home Nurses does an RN Evaluation to do initial assessment/assess home care needs then we check insurance coverage.

We then staff with appropriate staff (either licensed or non-medical) for interviews and/or to provide service per MD/patient. We are open 7AM to 7PM Mon- Fri plus on call 24 hours/day.

We are a member of the American Board of Home Care & CAHSAH certified.
